Functional Behavioral Assessments
We specialize in Functional Behavior Assessments (FBAs) in rural areas, supporting individuals with developmental disabilities. Each Consultive Assessment is strength-based, and person-centered which highlights each person's abilities and fosters meaningful progress.
We offer comprehensive services, including FBAs, Behavior Support Plans (BSPs), and Safety Plans, tailored to promote resilience and positive outcomes. Our support extends beyond assessment, providing ongoing consultation to ensure effective implementation.
ADA Paratransit Eligibility Determination
Paratransit determinations evaluate whether a person’s disability prevents them from independently using regular city buses. Through an application and sometimes an interview or medical verification, eligibility is classified as unconditional (always eligible), conditional (eligible only in certain situations), or temporary (eligible for a limited time). This process ensures services are reserved for those whose disabilities truly limit fixed-route access.
Risk Tier Consultation
We partner with agencies and families to develop individualized strategies for people on risk tiers, focusing both on the person’s unique needs and on building the skill and confidence of their Direct Support Professionals. Through a comprehensive review of behavioral data, environmental factors, and staff competencies, we craft person-centered plans that progressively reduce restrictions. Our ultimate aim is to safely exit individuals from the Risk Tier, so they can fully participate and thrive in the community they’ve chosen—at the lowest level of supervision necessary.
Psychosexual Risk Assessment
We provide trauma-informed, developmentally appropriate assessments that balance each youth’s right to restoration and dignity with the imperative of community safety. By integrating clinical interviews, validated risk-need tools, and collateral information (e.g., family, school, probation), we identify:
Protective factors and strengths to support healthy development
Dynamic risk indicators requiring targeted intervention
Environmental contributors that can be modified to reduce risk
From these data, we collaborate with caregivers and providers to design individualized treatment and supervision plans. Our recommendations emphasize skill-building, restorative opportunities, and community reintegration—while ensuring structured safeguards that protect both the young person and those around them.
Threat Assessment
With nearly a decade of experience in behavioral threat assessment, Dr. Zak provides high-impact consultation and strategic support to healthcare systems, law enforcement agencies, and community organizations. His work spans dynamic mitigation of complex cases; including targeted violence, suicidal risk, mass casualty prevention, insider threats; across clinical, educational, and public safety domains.
Workplace Violence Prevention
As a regional consultant in Workplace Violence Prevention, Dr. Zak has guided organizations in the development and implementation of Threat Assessment Teams (TATs) in accordance with Joint Commission standards for accreditation. His expertise includes policy design, multidisciplinary coordination, and sustainable intervention protocols that prioritize early identification and de-escalation of risk. Our services include:

Clinical Supervision
I provide clinical supervision in accordance with the State of Nebraska regulations governing the practice of mental health professionals, including supervision for Provisionally Licensed Mental Health Practitioners (PLMHPs) working toward independent licensure as LIMHPs or LMHPs. Supervision is structured to meet all statutory and regulatory requirements including competencies in diagnosis, treatment planning, risk assessment, and harm reduction.
I focused my postgraduate research on the complex nexus of risk encountered by law enforcement when engaging individuals in crisis. My work centers on the prevention of Disruptive Behaviors and high-risk Casualty Events with a specific emphasis on risks emerging in healthcare and social service environments. As a former Violence Prevention Manager for the Federal Government, I provided consultation to emergency departments and law enforcement agencies at the Federal, State, County, and Municipal levels. I also had the opportunity to serve as a member of the State of Nebraska Forensic Treatment Team at the Lincoln Regional Center, to co-respond with local police, and to serve on the Omaha Police Department (OPD) Threat Assessment Team, supporting field-based risk mitigation strategies in real time.
I earned my doctorate from Nebraska Methodist College, where I focused on the intersection of behavioral health, public safety, and systems-level risk mitigation. Currently, I am developing course content for educational systems focused on Mental Health, HIPAA (privacy exceptions), Duty to Warn/Duty to Protect and developing advanced training in threat assessment, disruptive behavior management, and violence prevention. I am a member of the State of Nebraska Threat Assessment Cadre, a past member of the Region 6 Behavioral Health Task Force, and a presenter at the International Co-Responder Conference, where I have shared cross-disciplinary strategies for preventing violence and enhancing public safety.
In my practice, I serve as a Consultative Assessment Clinician contracted with the State of Nebraska, specializing in the assessment and mitigation of Affective Violence—emotionally-driven, high-risk behaviors that threaten public safety and service access. This is a central focus of my clinical work. I also provide Behavioral Risk Assessments for Predatory, Affective, and Targeted Violence, particularly where third-party risk is involved. I am a certified sexual violence assessor and provide psychosexual evaluations for the juvenile population, providing clarity of risk factors to both court-related determinations and rehabilitative planning.
Additionally, I continue to consult with healthcare systems, law enforcement, and social service agencies on the management of Disruptive Behaviors, helping ensure individuals in crisis retain access to care in ways that are safe, respectful, and clinically sound for all involved. I have provided specialized trainings across disciplines, including healthcare settings, law enforcement, and behavioral health agencies. I am an active member of the Association of Threat Assessment Professionals.
